Starter Packet Request Form

We’re happy to mail our starter packet to you.  Please fill out the following form, and we’ll get the packet in the mail to you!  This form creates a mailing label for your packet, so please spell and capitalize properly and use the two-letter postal code (AL, for example) to avoid delivery problems.  Please allow two weeks for the packet to arrive.
